INSURANCE INFORMATION

INSURANCE FILING POLICIES

In order to serve you well, we would like to inform you of our office policies regarding the filing of insurance claims. We have made prior arrangements with many insurers and health plans to accept an assignment of benefits for several of our wellness services. This means we will bill those health plans for which we have an agreement and will only require you to pay your co-payment at the time of service. While we are pleased to be able to provide service to you, it is very difficult to keep track of all the individual requirements. We recommend that you contact your insurance company or human resources at your place of employment with specific questions regarding your plan benefits.

If at any time there is a change in your insurance plan, please notify the front office staff so that we may file the insurance claims on your behalf in a timely manner.

PRESCRIPTION REFILLS

Please contact your pharmacy to request a refill prior to calling our office. Drug prescription refills are filled only during office hours when the physician and nurse have access to your medical record. Please have the phone number of the pharmacy of your choice available at the time of your call.
